Kindergarten Registration

Registration for 2012-2013 incoming Kindergarteners will be Tuesday, February 21st at 934 SW Clay St.

Please bring the following:

Completed forms: (These are in the packet that was mailed to you)	Also, please bring:
student registration form	a state-certified Birth Certificate
family information form (if new to our school)	Baptismal Certificate
school health form	Social Security card or number
emergency contact form	$200.00 non-refundable fee (to hold a spot for your child next fall).

Be sure to make an appointment at registration for Kindergarten Screening on Friday, March 3rd.

KCI Immunization Form and physical form will be due on
the first day of school in August.
